Comprehensive Annual Fee Includes:
- Yearly comprehensive physical plus up to 11 additional office visits (up to 12 total office visits per year, $150 for each visit over 12)
- For patients who require home care, each home visit counts as 2 office visits (up to 6 home visits per year for exclusive home care, $300 for each additional visit)
- Coordination of your care with specialists and other providers
- Coordination of inpatient and rehabilitation care
- Review of test results and consults from other providers
- Annual flu shot
- EKG and gynecologic exam as medically indicated
- 24-hour direct access to Dr. Faraday by cell phone (except during vacations/continuing medical education, where coverage will be provided by another board-certified physician)
- Same or next day appointments when sick
- Unhurried appointments
- Short in-office wait time
- No additional paperwork sent to you or your insurance company

Patients retain their primary insurance to cover costs of laboratory and diagnostic tests, pharmaceuticals, and other medical services in accordance with the benefits described by their plan.
